Debt Fairness Act Notice

I understand that if my account balance remains unpaid after 3 statements, a letter will be sent to me notifying me that my account may be referred to a collection agency. I am responsible for paying any remaining balance to them. I acknowledge that in accordance with the Minnesota Debt Fairness Act (MN Stat. §507.09), effective October 1, 2024, Eye Associates of Central Minnesota will not withhold medically necessary care due to unpaid debt.
